Effective Immediately . . .
While on scene, please make every effort to gather all medications your patient is taking. If
there are more than three (3) . . .take the actual bottles to the ED. Place them in a Ziploc baggie,
if available, or any other bag type item.

If less than three (3) medications . . . make sure the name of the drug (spelled correctly), strength
and dosage is documented and given to the ED with the patient report.
